Abstract

Renal dialysis is a vital therapy for end-stage renal disease (ESRD) but presents significant clinical, economic, and humanistic challenges. This chapter highlights the critical role of pharmacists in improving outcomes for dialysis patients through evidence-based care. Pharmacists contribute to the optimization of pharmacotherapy, monitoring for drug-related problems, and educating patients on complex treatment regimens. Clinically, their interventions help manage comorbidities, prevent complications, and enhance adherence, ultimately improving patient safety and quality of life. Economically, pharmacists reduce healthcare costs by promoting cost-effective therapies, minimizing hospitalizations, and optimizing resource use. From a humanistic perspective, they empower patients through personalized counseling and education, improving satisfaction and treatment experiences. The chapter also reviews research supporting the effectiveness of pharmacist care in renal dialysis and discusses strategies to enhance their impact, including interprofessional collaboration, 192 digital health integration, and overcoming systemic barriers. Pharmacists are indispensable in delivering comprehensive, patient-centered dialysis care that is both clinically effective and economically sustainable.
