Evidence-Based Pharmacist Care (Clinical, Economic, and Humanistic Impact): Inflammatory Bowel Disease
Abstract
Inflammatory Bowel Disease (IBD), encompassing Crohn’s disease and ulcerative colitis, is a chronic, relapsing inflammatory condition of the gastrointestinal tract that imposes substantial clinical, economic, and humanistic burdens. This chapter explores the essential role of evidence-based pharmacist care in the comprehensive management of IBD. Clinically, pharmacists contribute to early identification, therapeutic drug monitoring, medication optimization, side effect management, and the prevention of complications. They also play a key role in educating 60 patients on treatment regimens, adherence strategies, and lifestyle modifications. Economically, pharmacist interventions reduce healthcare costs by improving medication adherence, minimizing hospital admissions, and ensuring the rational use of biologics and immunosuppressants. Humanistically, pharmacist involvement enhances patients’ quality of life by addressing psychosocial needs, reducing symptom burden, and promoting self-management. The chapter also discusses the facilitators and barriers to pharmacist-led care, integration of digital tools, and the evidence supporting pharmacist contributions to improved IBD outcomes. This comprehensive approach underscores pharmacists as integral members of the multidisciplinary team in delivering high-quality, patient-centered care for individuals with IBD.
