Abstract

Portal hypertension and cirrhosis are advanced liver conditions associated with significant morbidity, mortality, and healthcare burdens worldwide. This chapter explores the pivotal role of pharmacists in the evidence-based management of these conditions, emphasizing their clinical, economic, and humanistic impact. Clinically, pharmacists contribute to early detection, optimization of pharmacologic, prevention of complications and comprehensive patient monitoring. Economically, pharmacistled interventions improve medication cost-efficiency, reduce hospital readmissions, and decrease healthcare expenditures through appropriate therapy management and adherence support. Humanistically, pharmacists enhance patient quality of life by providing individualized education, improving medication adherence, supporting lifestyle modifications, and empowering patients in managing complex treatment regimens. 128 The chapter also highlights current research, identifies facilitators and barriers to pharmacist involvement, and presents digital innovations that support pharmacist roles in cirrhosis and portal hypertension care. This evidence-based approach underscores the essential contribution of pharmacists to safe, effective, and patient-centered liver disease management.
