Evidence-Based Pharmacist Care (Clinical, Economic, and Humanistic Impact): Sickle Cell Disease
Abstract
Sickle Cell Disease (SCD) is a chronic, inherited hemoglobinopathy that causes significant clinical complications, economic burden, and humanistic challenges. This chapter presents the multifaceted role of evidence-based pharmacist care in improving outcomes for individuals living with SCD. Clinically, pharmacists contribute to the prevention of complications, symptom control, medication optimization, pain management, and patient education, ultimately reducing morbidity and mortality. Their interventions improve treatment adherence, minimize drug-related problems, and support disease-modifying therapies. Economically, pharmacist-led care reduces healthcare costs by optimizing medication use, preventing hospitalizations, reducing medication waste, and enhancing transitions of care. Pharmacists also promote cost-effective strategies such as therapeutic drug monitoring, use of generics, and telepharmacy services. From a humanistic perspective, pharmacists enhance the quality of life for SCD patients by improving health literacy, promoting medication adherence, 124 reducing stigma, and empowering self-care through continuous education and counseling. The chapter underscores the need for integrating pharmacists into multidisciplinary care teams and leveraging technology and policy reforms to expand their impact in managing SCD across clinical, economic, and humanistic domains.
