Abstract

This chapter explores the crucial role of evidence-based pharmacist care in identifying, preventing, and managing patient care errors and related problems. Pharmacists are integral to minimizing medication errors across the continuum of care—from prescribing and dispensing to administration and monitoring. Their interventions significantly improve patient outcomes through medication reconciliation, patient counseling, and chronic disease management. The chapter discusses the clinical, economic, and humanistic impacts of pharmacist care, including reduced adverse drug events, improved adherence, lowered healthcare costs, and enhanced patient satisfaction. It also highlights key facilitators, such as collaborative practice agreements and clinical decision support systems, while addressing common barriers, including regulatory constraints and limited integration in care teams. The chapter emphasizes that empowering pharmacists and addressing systemic challenges are essential to enhancing healthcare safety, efficiency, and quality through evidence-based pharmacy practice.
