Abstract

This chapter highlights the essential role of evidence-based pharmacist care in enhancing medication adherence, compliance, concordance, and reconciliation. Pharmacists are uniquely positioned to educate patients, promote shared decision-making, and identify barriers to optimal medication use. Their interventions significantly improve clinical outcomes, reduce hospital readmissions, and prevent medication-related problems. The chapter examines the clinical, economic, and humanistic impacts of pharmacist involvement, emphasizing improved therapeutic success, reduced healthcare costs, and enhanced patient empowerment and satisfaction. It also explores facilitators such as integrated healthcare systems and technology, while addressing persistent barriers including lack of provider recognition, reimbursement issues, and limited access to health records. Ultimately, the chapter calls for systemic reforms and wider 118 integration of pharmacist-led services to optimize medication therapy, improve patient outcomes, and support a more patient-centered, efficient healthcare system.
