Abstract

This chapter explores the pivotal role of pharmacists in health screening and disease prevention through an evidence-based care framework. Pharmacists, owing to their accessibility and medication expertise, are uniquely positioned to deliver preventive services such as point-of-care testing, risk assessment, chronic disease management, immunization, and lifestyle counseling. The chapter highlights the clinical, economic, and humanistic impacts of pharmacist-led interventions—ranging from improved access and cost-effectiveness to enhanced quality of life and patient empowerment. Key facilitators and barriers to implementation are analyzed, alongside strategic recommendations for integrating evidencebased practices, leveraging digital health tools, and expanding pharmacist competencies. Ultimately, the chapter reinforces the essential role of pharmacists in modern healthcare systems as proactive, patient-centered providers of preventive care, contributing to improved health outcomes and reduced disease burden.
