Evidence-Based Pharmacist Care: Pharmacy Stewardship Programs
Abstract
This chapter explores the diverse and critical role of pharmacists in pharmacy stewardship programs aimed at optimizing medication use, enhancing patient safety, and ensuring cost-effective healthcare delivery. These programs include antimicrobial, opioid, and medication safety stewardship, as well as medication therapy management, pharmacoeconomics, chronic disease management, immunization, and transitions of care. Pharmacists contribute to improved therapeutic outcomes, reduced adverse drug events, lower healthcare costs, and enhanced patient engagement through evidence-based interventions. The chapter evaluates the clinical, economic, and humanistic impacts of pharmacist-led stewardship initiatives and emphasizes their value in addressing medication-related challenges, minimizing antimicrobial resistance, and promoting responsible prescribing. It also examines key facilitators such as technology, education, and interprofessional collaboration, alongside barriers including limited authority, regulatory constraints, and resource shortages. The chapter concludes by advocating for stronger policy support, continuous training, and systemic integration of pharmacists into stewardship programs to advance patient-centered, sustainable, and resilient healthcare systems.
