Abstract

This chapter outlines the pivotal role of pharmacists in anticoagulation clinics, emphasizing their contributions to safe, effective, and patientcentered anticoagulant therapy. Pharmacists are instrumental in patient education, medication management, dosing adjustments, and monitoring therapies such as warfarin and direct oral anticoagulants. Their involvement enhances therapeutic outcomes by reducing bleeding risks, preventing thromboembolic events, and improving adherence. The chapter explores the clinical, economic, and humanistic impacts of pharmacist-led anticoagulation services, highlighting improvements in INR control, reduced hospital readmissions, cost savings, and enhanced patient quality of life. It also addresses facilitators—including interprofessional collaboration, access to point-of-care testing, and evidence-based protocols—as well as barriers such as limited reimbursement, scope-of-practice constraints, and patient adherence issues. By integrating pharmacists into multidisciplinary care teams and addressing systemic challenges, anticoagulation clinics can deliver high-quality, cost-effective care that meets the growing needs of patients requiring long-term anticoagulation.
