Evidence-Based Pharmacist Care Skills and Competencies: Hematological Care
Abstract
Pharmacists are integral to the multidisciplinary management of hematological disorders, providing expertise in medication optimization, anticoagulation therapy, chemotherapy preparation, patient education, and supportive care. This chapter outlines the expanding roles and essential competencies required for pharmacists to deliver evidence-based hematological care. Key responsibilities include therapeutic drug monitoring, infection prophylaxis, pain and anemia management, clinical research involvement, and collaboration with hematology teams. The chapter also emphasizes the importance of clinical knowledge, pharmacovigilance, communication skills, personalized medicine, informatics, regulatory awareness, and interprofessional collaboration. Recommendations for enhancing pharmacist competencies include continuing education, clinical experience, mentorship, simulation, and participation in research and quality improvement initiatives. By embracing these competencies, pharmacists can significantly improve clinical, economic, and humanistic outcomes in patients with hematological conditions, while advancing pharmacy practice in hematology through innovation and patient-centered care.
