Evidence-Based Pharmacist Care Skills and Competencies: Special Population Care
Abstract
Pharmacists play a critical role in delivering evidence-based care to special populations, including children, the elderly, pregnant women, individuals with disabilities, and those with chronic or complex conditions. This chapter outlines the essential skills and competencies required to provide safe, effective, and personalized care to these groups. Key responsibilities include medication management, dose adjustments, patient education, preventive care, cultural competence, and advocacy. The chapter also emphasizes the importance of interprofessional collaboration, ethical decision-making, pharmacovigilance, and integration of digital health tools. Detailed recommendations are provided to enhance pharmacist competencies through continuing education, certification, simulation training, research participation, and quality improvement initiatives. By adopting a patient-centered approach and continually updating their knowledge and practices, pharmacists can significantly improve therapeutic outcomes, reduce health disparities, and promote equitable healthcare access for special populations.
