Alternative Treatment of Refractory Plummer-Vinson Syndrome with Esophageal Web Using IT2 Knife Endoscopic Mucosal Incision: A Case Report
Abstract
Plummer-Vinson syndrome (PVS), characterized by dysphagia, iron deficiency anemia, and esophageal webs, is usually treated with iron supplementation and endoscopic dilatation. However, some patients remain refractory to standard dilatation therapy. We report the case of a 46-year-old woman with recurrent dysphagia due to PVS, unresponsive to repeated bougie and balloon dilatations. Endoscopic evaluation revealed an esophageal web, which was successfully treated with an insulated-tipped (IT2) knife mucosal incision. The patient remained symptom-free during follow-up without complications. IT2 knife mucosal incision appears to be a safe and effective alternative for refractory esophageal webs in PVS, providing durable symptom relief in selected patients.
