Abstract

This guideline was developed as a result of a pilot project to introduce a holacracy at a primary care site with independent patient admissions by a district nurse. It contains a template for the form to be completed by a nurse during an independent patient admission and the procedure for completing it. The use of this guideline by personnel of any other facilities is not limited. Responsibility for its correct use lies with the person using it.
