Abstract

Pyoderma remains one of the most common causes for patient visit in outpatient practice of dermatovenereologists and cosmetologists due to its clinical polymorphism, tendency to recurrence and the risk of developing cosmetic defects. At the same time, there are difficulties in making decision on which particular antibacterial drug to use as an empirical therapy. The urgency of the problem is emphasized by growing antibiotic resistance, comorbidities associated with impaired skin barrier and increased patient aesthetic expectations. Within the frame of the article, modern approaches to treatment with a focus on rational choice of topical antibacterial agents are considered. Comprehensive analysis of literature and own clinical experience aimed at assessing the effectiveness, safety and resistance profile of the main topical antibacterial drugs used in outpatient dermatological practice was carried out. The presented data and a review of clinical cases demonstrate that personalized management of patients with consideration of the characteristics of the infectious process course can increase the effectiveness of treatment, reduce the recurrence rate and improve clinical and aesthetic outcomes. Currently, in pediatric and adult patients, the leading role is assigned to a hydrophilic mupirocin 2% ointment, which can be used as a combination treatment with modern antiseptic solutions and systemic antibacterial agents, if necessary, to ensure high clinical effectiveness and contribute to a significant reduction in the duration of therapy.
