Technologies for Patient Navigation and Care Coordination in Older Adults

This scoping review protocol outlines a strategy to map digital technologies used for older patient navigation during care transitions, focusing on coordination and access. Guided by the BEAN theoretical framework, it examines how individual, technological, and ecological factors influence the integration of care. The findings aim to identify barriers and facilitators to optimize health management and continuity of care for the aging population.
