Exploring home-based network care for frail older adults with cardiovascular disease: a scoping review protocol
Abstract
Objective: This scoping review aims to systematically identify the components of home-based network care for frail older adults with cardiovascular disease and to explore barriers and facilitators to its implementation. Introduction: To improve health outcomes for frail older people with cardiovascular disease, care must shift from a condition-specific approach to integrated, person-centred, home-based care that also addresses psychosocial and functional needs. Such care requires close collaboration between hospital-based and community-based healthcare providers, yet the essential components of an effective home-based care network remain unclear. Inclusion criteria: Older adults aged 65 years and older diagnosed with cardiovascular disease and living at home who receive any form of home-based network care. Methods: The scoping review will be conducted in accordance with the framework of Arksey and O’Malley’s, as further refined by the Joanna Briggs Institute. Searches will be performed in PubMed, Embase, and CINAHL, complemented by a screening of relevant grey literature. All study designs will be considered eligible. Study selection and data extraction will be carried out independently by two researchers. Both quantitative and qualitative data analysis will be performed. The review will adhere to the Preferred Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews.
