Abstract

Chronic diseases represent a growing burden for health systems and require sustained strategies to support self-management and active disease management in ambulatory and home-based settings. In recent years, digital health has driven interventions such asmobile applications, mHealth platforms, chatbots, and remote monitoring tools; the incorporation of artificial intelligence (AI) has expanded their potential through personalization, pattern analysis, and adaptive feedback. However, the available evidenceis heterogeneous and dispersed, and therefore the purpose of this review is to systematically map the literature on AI-enabled digital interventions applied to self-management and active management of chronic diseases in adults with diabetes, cardiovascular disease, and asthma/COPD, describing their characteristics, applications, reported benefits, and implementation challenge.
