DOUBLE-LUMEN ENDOTRACHEAL TUBE INTUBATION IN A PATIENT WITH SITUS INVERSUS TOTALIS
Abstract
Situs inversus totalis (SIT) is a rare congenital syndrome in which the normal positions of the thoracic and abdominal organs are displaced in a mirror image manner. These patients generally do not experience any problems in their normal lives, but when a surgical intervention is necessary, they may cause technical difficulties for the surgical team. In this case, we wanted to share our anesthesia approach to the patient with SIT who developed spontaneous pneumothorax (px) and was taken to bullectomy operation. Although we came across many cases regarding anesthesia management in patients with situs inversus in our literature review, it was observed that the cases in which a double lumen tube was applied were very few. Although there is no different approach to anesthesia, special attention should be paid to monitoring, the bronchial tree carefully evaluated before intubation, and an appropriate double-lumen endotracheal tube should be used.
