Comprehensive clinical spectrum of dengue: Integrating hematological and hemodynamic parameters
Abstract
Background: The classic sign of dengue hemorrhagic fever (DHF) is still fluid leaking. Since the currently recommended predictors of DHF for adults with dengue are based on studies done on children, their applicability is unclear. This study aimed to determine the prevalence of third space accumulation in Dengue fever and it's co-relation with age, sex and dengue serology and hemodynamic status.Materials and Methods: A prospective study was conducted on 40 adult dengue patients, who were >18 years of age, admitted during period of July 2023 to September 2023. Chi-Square analysis was carried out to assess the corelation. Results: A study on 40 confirmed dengue patients aged over 18 revealed significant correlations between serological markers and clinical manifestations. Among them, 27 tested positive for NS1 antibodies, 2 for IgG antibodies, and 11 for IgM antibodies. Thrombocytopenia was observed in 17 patients, with a significant association with both pleural effusion (chi-sq = 9.598, p= 0.00194) and ascites (chi-sq = 7.5192, p=0.006105). The study underscores the importance of considering these associations in the clinical management of dengue patients. Conclusion: This study establishes a significant correlation between thrombocytopenia and third space fluid collection, specifically pleural effusion and ascites, in dengue patients. The findings enhance our understanding of hematological and fluid dynamics in dengue infection, emphasizing the importance of comprehensive management strategies and hospital attention for affected individuals.
