Abstract

The orthodontic headgear is one of the most enduring and versatile orthopedic appliances used for correcting sagittal skeletal and dental discrepancies, primarily in growing Class II patients. Its mechanism relies on applying Extra-Oral Force (EOF) to the maxilla to restrict or redirect its growth and/or distalize the maxillary dentition. While its routine use declined following the advent of fixed functional appliances, recent systematic reviews affirm its strong, evidencebased efficacy, particularly in achieving true orthopedic restraint of the maxilla. This comprehensive review aims to trace the historic evolution of the appliance, meticulously dissect its complex biomechanical force systems using vector analysis, detail its clinical application based on vertical facial pattern, and integrate recent literature regarding its efficacy, safety, and compliance monitoring in contemporary orthodontic practice.
