Classification of Complications Post Pancreaticoduodenectomy According to Modified Clavien-Dindo Score
Abstract
Background: Pancreaticoduodenectomy (PD) is regarded as the preferred intervention for a variety of benign and malignant cancers of the pancreatic head or periampullary region. Aim: To retrospectively classify post-PD complications according to the modified Clavien Dindo classification score. Methods: This retrospective cohort study was carried out on 404 cases that underwent PD at the National Liver Institute over a ten-year period till March 2023. PD complications were assessed according to the modified Clavien Dindo classification score. Results: Surgical re-operation under general anesthesia (grade III b) was done in 81 (20%) of patients. Early postoperative mortality (death within the first month) occurred in 27 (6.7%) of the study population including 12 (2.9%) patients due to sepsis and multi-organ failure after POPF and subsequent complications, 4 (1%) mortalities occurred after reoperation from PPH, 5 (1.1%) mortalities occurred after sudden massive PPH without prior reoperation, 1 (0.2%) patient due to cerebral stroke, 3 (0.7%) patient due to myocardial infraction, and 2 (0.4% ) patient from massive pulmonary embolism. Conclusions: The adoption of standardized definitions by the ISGPS and the Clavien–Dindo classification has enabled accurate assessment and fair comparison of outcomes across centers.
