Abstract

Objectives: Tibial deformities are thought to be a major contributing factor to osteoarthritis in the knee and ankle. Tibial osteotomy aims to achieve ideal leg alignment by reconstructing the tibia in the proper biomechanical axis, hence preventing knee joint overload. Our study aimed to detect the precision of correction of multiapical and multidirectional tibial abnormalities by using the Deftfix fixator frame (DFF).Methods: Prospective case-control pilot research was conducted from September 2023 to March 2025 on 10 patients (16 limbs) who had corrective tibial osteotomy using a DFF after presenting with tibial deformity predominantly caused by Blount disease (75 %). Every patient underwent a preoperative clinical and radiological evaluation, and they were monitored for a full year. Results: The patients were 22.8 years old on average, the majority were men, and the main complaint was long-standing leg deformities. Our findings at the final follow-up indicated that DFF was highly effective in the treatment of tibial deformities. There was a statistically significant improvement in MPTA, with mean corrections of 15.2°. In terms of clinical evaluation, the ASAMI bone and functional scores were excellent in 93.3% of patients and good in 6.7%.Conclusion: DFF is a new frame with a promising result. Our results considered a valid option for correction of tibial deformities, which allows simultaneous multidirectional correction, the ability to correct any residual deformity until achieving optimal mechanical joint orientation, and creates the strut adjustment prescription regimen, which was handled quite smoothly by the patients, following the same principles of callus distraction.
