Abstract

Background: Ischemic left ventricular systolic dysfunction (LVSD) constitutes substantial clinical burden worldwide and linked to complications and impaired life quality. Contemporary therapeutic approaches aim to address both ischemic symptoms and heart failure manifestations in order to improve clinical outcomes and patient well-being. Objectives: evaluating short-term changes in life quality and major cardiac events among patients with ischemic LVSD managed with different treatment strategies. Methods:128 patients with ischemic LVSD enrolled from two tertiary care centers in Egypt. Patients were managed either with revascularization or medical therapy. The revascularization cohort included patients undergoing percutaneous coronary intervention (29 patients) or coronary artery bypass grafting (35 patients). Health-related life quality studied using the Seattle Angina Questionnaire (SAQ) and the Kansas City Cardiomyopathy Questionnaire (KCCQ). Cardiac events were documented over 90 days. Results: demographics, co-morbidities, mean ejection fraction were comparable between groups. SAQ scores improved significantly in the revascularization arms, with PCI increasing from 67.6 ± 10.5 at 1 month to 77.8 ± 10.2 at 3 months (p < 0.001) and CABG from 69.1 ± 9.0 to 79.0 ± 8.8. GDMT showed smaller improvements (50.2 ± 8.5 to 57.6 ± 8.6). In contrast, KCCQ scores demonstrated no significant differences between groups over time. Short-term MACEs were similar across all treatment strategies. Conclusions: At three months, revascularization significantly improved angina-related HRQoL without affecting heart failure–related quality of life, while early MACEs were comparable between revascularization and GDMT.
