تطوير مقياس تقدير الاضطراب ثنائي القطب لدى البالغين المترددين على العيادات الخارجية المتخصصة باستخدام الشبكات العصبية الاصطناعية - Developing a bipolar disorder rating scale for outpatient adults using artificial neural networks
Abstract
Abstract: This study aimed to develop a bipolar disorder (BD) rating scale for adults by employing artificial neural networks to identify the most influential elements in the conceptual framework of BD. The research used a descriptive methodology to achieve these objectives. The sample consisted of 358 adults (196 males and 162 females) attending specialized outpatient mental health clinics. Their ages ranged from 21 to 53 years (34.61±8.55). Of these, 53 patients were diagnosed with BD (14 with type I and 39 with type II). The BD rating scale (developed by the researcher) was administered to them. Data were analyzed using JASP 0.19.2.0 and MedCalc 23.4.0 software. The results of the confirmatory factor analysis demonstrated the good fit of the two-factor model of the BD rating scale, composed of mania and depression, with the sample data. The results of the artificial neural network analysis also showed a set of items that were more intermediate and potentially influential in the conceptual structure of BD, as items (7, 8, 9, 2, 10) stood out in the conceptual structure of mania, while items (5, 4, 10, 6, 1, 9) stood out as having the highest influence in the conceptual structure of depression. The results also showed that the scale exhibits a high degree of internal consistency for the manic and depressive factors, along with good reliability indicators. Cronbach's alpha coefficients for the manic and depressive factors were 0.904 and 0.942, respectively, while the McDonald Omega coefficients were 0.902 and 0.942. Furthermore, the results indicated that the optimal cutoff score for detecting a manic episode is a score greater than 19 on the manic factor, with a sensitivity level of 80.00% and a specificity of 95.83%. The optimal cutoff score for detecting a major depressive episode is a score greater than 20 on the depressive factor, with a sensitivity level of 87.50% and a specificity of 93.33%. These results suggest that the scale possesses high validity and reliability indicators for use in both clinical and non-clinical samples to detect BD in adults. In light of these results, the research recommends the necessity of applying this scale to detect BD and classify those affected by it and those not affected. It also recommends the importance of employing artificial neural networks in developing psychological scales and detecting the interconnectivity, convergence, strength, and potential impact of the items that make up the psychological scales.
