Abstract

Abstract Inappropriate sexual behaviors among adolescents with autism spectrum disorder (ASD) pose significant clinical and social challenges, particularly in culturally conservative contexts where validated, culturally adapted assessment tools are scarce. This study reports the development and psychometric validation of the Concerning Sexual Behaviors Scale for Children with Autism Spectrum Disorder (CSB-ASD)—a novel, Arabic-language instrument designed to assess concerning sexual behaviors in youth with ASD aged 10–18 years. A cross-sectional sample of 440 participants was recruited from Egypt, Saudi Arabia, Jordan, and the United Arab Emirates, including 320 youth with ASD, 40 with ADHD, 40 with conduct disorder, and 40 typically developing controls. Data were collected from three independent informants (parents, teachers, and clinicians). Confirmatory factor analysis supported an 8-factor structure with excellent model fit (CFI = 0.93, RMSEA = 0.052). The scale demonstrated high internal consistency (α = 0.93), strong inter-rater reliability (ICC = 0.89), and test–retest stability (r = 0.86). Convergent validity was evidenced by a robust correlation with the Child Sexual Behavior Inventory (r = 0.76, p < 0.001). Critically, the CSB-ASD exhibited strong discriminant validity, significantly differentiating the ASD group from all comparison groups (F = 38.72, p < 0.001), particularly on subscales reflecting impaired sexual understanding and social-contextual awareness. Six items were empirically identified as “risk indicators” warranting immediate clinical intervention. The CSB-ASD represents the first culturally sensitive, psychometrically sound tool for assessing concerning sexual behaviors in Arabic-speaking youth with ASD, offering a practical framework for early identification, multidisciplinary intervention, and ethical clinical decision-making in understudied populations. Keywords: autism spectrum disorder; concerning sexual behaviors; behavioral assessment; cultural adaptation; discriminant validity; Arabic-speaking populations; psychometric validation.
