Financial Sustainability and Reserve Adequacy of Egypt’s Universal Health Insurance System: A Stochastic Risk-Based Assessment
Abstract
The expansion of universal health insurance systems in emerging economies presents significant challenges related to long-term financial sustainability and reserve adequacy. This research evaluates the financial dynamics of Egypt’s Universal Health Insurance System (UHIS) using an integrated actuarial and stochastic modeling framework. Building on earlier deterministic projections, the analysis incorporates updated macroeconomic conditions, healthcare utilization behavior, and enrollment dynamics, while explicitly capturing uncertainty through Monte Carlo simulation. The results show that early operating surpluses and reserve accumulation provide only a temporary buffer against rising expenditure pressures. As coverage matures and healthcare utilization intensifies, the system shifts toward structural operating deficits, reflecting a persistent imbalance between revenues and expenditures. Risk-based outcomes indicate that financial stress emerges across a broad range of plausible scenarios rather than being limited to extreme cases. Although accumulated reserves initially absorb operating shortfalls, their depletion accelerates once deficits persist, highlighting the limitations of reserve-based stabilization in the absence of structural adjustments.Sensitivity analysis identifies demand-side factors; particularly growth in healthcare utilization; as the dominant source of financial vulnerability, while revenue-side interventions offer only partial and short-lived mitigation. These findings suggest that long-term sustainability depends less on the magnitude of initial financial buffers and more on the system’s capacity to manage utilization, contain costs, and adapt to evolving risks. From a policy perspective, the research highlights the need for a proactive, risk-informed governance framework integrating utilization management, strategic purchasing, and dynamic reserve monitoring to enhance the resilience of UHIS under fiscal constraints.
