Abstract

Background: Platelet activation and endothelial dysfunction play central roles in the cardiovascular complications of chronic kidney disease (CKD). Mean Platelet Volume (MPV) and adropin are emerging biomarkers that may reflect these interrelated processes in hemodialysis (HD) patients. This research aimed to evaluate the relationship between MPV, serum adropin levels, and inflammatory and metabolic markers in patients with CKD undergoing maintenance HD. Methods: This cross-sectional study included 86 adult HD patients. Clinical, anthropometric, and laboratory data were collected. Serum adropin was measured using enzyme-linked immunosorbent assay (ELISA). Correlations between MPV, adropin, and relevant clinical, biochemical, and dialysis parameters were analyzed. Results: MPV correlated positively with ferritin, ESR, CRP, and lipid parameters. (p < 0.05) but negatively with hemoglobin (p = 0.048) , Kt/V(p=0.044) and adropin (r = –0.248, p = 0.022). Adropin showed positive correlations with dialysis adequacy indices (Kt/V, URR, and vintage) and negative correlations with inflammatory markers and lipid profile components. MPV differed significantly across CKD etiologies (p = 0.032), with highest values in autoimmune disease. Patients with stroke had significantly lower adropin levels (p = 0.031). MPV > 7.5 fL predicted platelet activation with 78% sensitivity and 72% specificity, while adropin < 350 pg/mL predicted endothelial dysfunction with 80% sensitivity and 70% specificity. Conclusion: MPV and adropin serve as complementary markers reflecting platelet activation and endothelial function in HD patients. Elevated MPV indicates inflammatory and thrombotic risk, whereas reduced adropin reflects vascular impairment. Their combined evaluation may enhance cardiovascular risk assessment in CKD
