Laser Sphincterolysis versus Open Lateral Internal Sphincterotomy in Treatment of Chronic Anal Fissures.
Abstract
Background: Chronic anal fissure is a common and painful anorectal condition often requiring surgical intervention when conservative therapy fails. Laser sphincterolysis (LS) has emerged as a minimally invasive alternative to conventional open lateral internal sphincterotomy (LIS), offering potential advantages such as reduced bleeding, precise sphincter division, and faster recovery. This study aimed to compare between laser sphincterolysis and lateral open internal sphincterotomy in treatment of chronic anal fissure. Methods: This randomized controlled clinical trial was conducted in General Surgery Department, Faculty of Medicine, Zagazig University on 42 chronic anal fissure cases; divided into two groups; 21 cases underwent laser technique (group I) and 21 cases underwent open lateral internal sphincterotomy technique (group II). Every patient underwent baseline investigations, a thorough history collection, and a suitable local examination. Results: Laser sphincterolysis demonstrated significantly shorter operative time (14±3.5vs. 25±5.1min; p
