Abstract

BACKGROUND Online platforms such as Reddit are increasingly used to seek mental health information and peer support, particularly for depression and anxiety. These spaces offer anonymity and accessibility, which may be valuable for individuals who experience mental health stigma. However, concerns persist regarding the accuracy and safety of peer-generated advice, and empirical evidence on whether community-endorsed advice aligns with established clinical guidelines remains limited. OBJECTIVE This study examined the extent to which highly ranked peer advice on Reddit’s r/mentalhealth subreddit for depression and anxiety aligns with evidence-based international and Australian clinical guidelines. METHODS A cross-sectional qualitative content analysis was conducted on 1,065 publicly available Reddit posts related to depression and/or anxiety and their top-ranked advice. Posts were classified as containing action-based advice or non-action-based expressions using a large language model with manual verification. Action-based advice was thematically analyzed and mapped against recommendations from established national and international clinical guidelines, including NICE, WHO mhGAP, and Australian frameworks. RESULTS Among the dataset, 327 anxiety-related posts and 243 depression-related posts contained action-based advice. Six overarching advice categories were identified: (1) seeking professional help, (2) behavioral and lifestyle changes, (3) emotional regulation and cognitive strategies, (4) interpersonal and social dynamics, (5) self-prioritization and life direction, and (6) crisis and immediate safety. Seeking professional help was the most frequently endorsed category for both depression and anxiety. Overall, most action-based advice aligned with evidence-based guidelines, particularly recommendations related to professional help-seeking, behavioral activation, and emotional regulation. Partial or non-alignment was more common in complex or high-risk contexts, such as crisis situations or interpersonal conflict. CONCLUSIONS Highly ranked mental health advice on Reddit frequently aligns with evidence-based guidance, suggesting that community-endorsed content may offer meaningful and broadly appropriate support for individuals experiencing depression or anxiety. Nonetheless, limitations remain in complex and high-risk situations. These findings highlight the potential role of online peer-support platforms as complementary sources of guidance and underscore the need for continued research into their public health implications.
