Abstract

Background and Objective: According to statistical reports from the Population Monitoring Center of the National Organization for Civil Registration (NOCR), the crude death rate in Iran in 2023 was 3.5 per 1,000 population, which in Gilan Province with 4.7 and 3.9 per 1,000 in Kohgiluyeh and Boyer-Ahmad Province, representing the highest and lowest provincial death rates, respectively. This report indicates that there is still a lack of proper understanding among executive officials regarding the requirements for accurate comparisons. While the crude death rate may be useful for a specific province, the heterogeneous population distribution across provinces makes comparisons unreliable. To enable valid comparisons, age-standardized death rates should be used. This study aims to provide a reliable report on death rates in Iran and its provinces by sex. Materials and Methods: Raw and Age standardised rates death data due to all causes from 1980 to 2023 were extracted from the Global Burden of Disease (GBD) study. Crude and age-standardized death rates were reported by sex, province, and sociodemographic index (SDI). Results: In 2023, the crude and age-standardized death rates in Iran were 415.4 (400.8–427.8) and 549.8 (530.5–568.5) per 100,000 population, respectively. The age-standardized death rate was higher in men 619.7 (592.4–648.4) than in women 482.6 (457.5–509.6). The highest age-standardized death rates were observed in Qom, Zanjan, and East and West Azerbaijan provinces, while the lowest rates were reported in Tehran, Kohgiluyeh and Boyer-Ahmad, and Fars provinces. Conclusion: Contrary to the NOCR report, the highest age-standardized death rate in 2023 was not in Gilan Province but in Qom and Zanjan provinces. Similarly, the lowest rate was not in Kohgiluyeh and Boyer-Ahmad but in Tehran Province.
