Abstract

Oral Lichen Planus (OLP) is a chronic autoimmune mucocutaneous disorder often associated with systemic conditions and psychological stressors. This case report presents a 41-year-old male with a two-year history of oral lichen planus and hypothyroidism, along with a six-month history of anal fissure. The patient reported burning oral pain aggravated by spicy and sour foods, blackish discoloration of the left buccal mucosa, dryness of mouth, anxiety, forgetfulness, and lethargy. Previous corticosteroid therapy provided only temporary relief, followed by relapse. A detailed holistic assessment revealed significant emotional suppression, financial stress, early childhood trauma, and a chilly constitution with burning pains ameliorated by warmth. Miasmatic evaluation indicated a dominant psoro-syphilitic background with latent sycotic tendencies. Based on the totality of symptoms, Arsenicum album was prescribed as the constitutional remedy, followed by Sulphur as an intercurrent remedy to address underlying psoric tendencies. Over six months of treatment, the patient showed more than 90% healing of oral lesions, complete resolution of anal fissure, significant reduction in burning sensation, improvement in thyroid function (initial TSH 26 µIU/mL), and marked psychological stabilization. The case demonstrates the effectiveness of an individualized constitutional homeopathic approach in managing chronic autoimmune mucosal disorders, emphasizing the role of miasmatic understanding and psychosomatic integration in long-standing conditions such as OLP with hypothyroidism.
