PREDICTION OF ADVERSE MATERNAL OUTOCMES IN PRE-ECLAMPSIA USING FULLPIERS RISK REDICTION MODEL IN A TERTIARY CARE CENTRE.
Abstract
Objective – 1. Determine the likelihood of developing adverse maternal outcomes in pre eclampsia using fullPIERS caliculator. 2. To follow up and analyze the maternal outcome in pre eclampsia. 3. Research the fullPIERS model’s ability to foretell unfavorable outcomes for mothers with preeclampsia. Study design – prospective observational study Setting – Tertiary care Hospital Sample - Pregnant women of age 18-45 attending GMH, Tirupati with pre eclampsia are taken into the study. Sample size is calculated with a beta error of 0.84 and an alpha error of 1.96, the predicted sample size was 164 with 95% confidence interval, which gave the test 80% power. Methods - It is a prospective observational study on 164 antenatal mothers admitted in Government maternity hospital, Tirupati. After obtaining the patients’ permission, all necessary investigations were conducted and score them using the fullPIERS system. Patients will be followed up for complications like imminent eclampsia, eclampsia, HELLP syndrome, PRES syndrome and pulmonary oedema and the result will be compared to their fullPIERS score. Statistical analysis was performed using SPSS (Statistical Package for Social Sciences) software version 17. For categorical data, chi-square test will be used. For all the tests P-value of 0.05 or less will be considered for statistical significance. Sensitivity, specificity and positive likelihood ratios were done using medcalc software. Results - The study shows the fullPIERS model was effective in predicting the adverse maternal outcomes. Considerable association between high fullPIERS scoring and the adverse maternal outcomes had been found. But the fullPIERS model has got a poor negative predictive value. Despite of having 46 women with adverse maternal outcomes only 3 members had significant fullPIERS scoring. The scoring system had shown good specificity as all the 3 members with high fullPIERS scoring developed adverse maternal outcomes. Conclusion - Preeclampsia has a major effect on the health of both the mother and the fetus, s. Consistent trends in age distribution, gravida status, gestational age, blood pressure, SPO2 levels, laboratory findings, mode of delivery and maternal outcomes underscore the importance of effective management and early intervention in preeclampsia cases. The predictive utility of the FULLPIERS score is validated across multiple studies, emphasizing its role in guiding clinical decisions and improving outcomes. Through continuous monitoring, timely intervention, and comprehensive prenatal care, Preeclampsia risks may be reduced, improving mother’s health.
