Abstract
Objective: Septate uterus is one of the most common Müllerian anomalies and is associated with adverse reproductive outcomes such as infertility, recurrent pregnancy loss, preterm birth, and malpresentation.Although hysteroscopic septum resection is widely performed to improve reproductive outcomes, its effectiveness remains controversial, particularly across different patient subgroups. The aim of this study is to evaluate reproductive and obstetric outcomes after hysteroscopic septum resection in women managed at a tertiary referral center and to compare outcomes across reproductive subgroups. Material and Methods:This retrospective cohort study included 174 women who underwent hysteroscopic uterine septum resection between 2010 and 2020.Reproductive outcomes including pregnancy rate, live birth rate, mode of conception, and obstetric outcomes were evaluated.Subgroup analyses were performed according to infertility type and history of adverse pregnancy outcomes. Results: The overall pregnancy rate was 63.2% [110/174; 95% confidence interval (CI): 55.8-70.1],and the live birth rate was 56.9% (99/174; 95% CI: 49.5-64.1).Reproductive outcomes were comparable among women with primary infertility, secondary infertility, and those with a history of preterm delivery with or without abortion.Spontaneous conception occurred in 52.7% of pregnancies.Repeat hysteroscopy due to residual septum was required in 13.8% of patients and was not associated with significantly different reproductive outcomes.Among singleton live births, vertex presentation was observed in most cases, and cesarean delivery occurred in 62.6%.ÖZ Amaç: Septat uterus, infertilite, tekrarlayan gebelik kaybı, preterm doğum ve malprezentasyon gibi olumsuz üreme sonuçlarıyla ilişkili en sık Müllerian anomalilerden biridir.
