Congratulations to the founder and all winners of the MSD Health Prize 2025. Each of the teams presented a valuable idea. Now it is time to turn these ideas into solutions. However, attempts to implement these ideas are encountering a "Hippocratic problem" everywhere. Hippocrates (460– 370 BC) insisted that doctors should not just treat the patient's "sore knee", but should consider the patient as a whole. Every conscientious doctor tries to base their treatment strategy on their perception of the patient's "health risk profile". This unstructured (non-operationalised) orientation of the treatment strategy is perceived by experts as the "natural chaos" of everyday care [1]. The commentary describes the tasks of science in analysing and solving the consequences of this global chaos.
