Estrogen receptor signaling in post menopausal women : molecular mechanisms, pharmacological diversity and precision therapeutic strategies
Abstract
Oestrogen receptor (ER) signaling governs abroad network of physiological processes whose disruption at menopause precipitates osteoporosis, cardiovascular disease and cognitive decline. Despite decades of intensive research, hormone therapy outcomes remain profoun dly heterogeneous across individuals, and the molecular determinants of this variability are incompletely integrated into clinical decision - making. This review synthesises current mechanistic understanding of ER biology across five interconnected axes: lig and - specific conformational dynamics of helix - 12 in the ligand - binding domain; the RANKL/OPG osteoclastogenic cascade whose ERα - dependent set - point controls skeletal remodelling and fracture risk; the dual genomic and non - genomic eNOSsignalling pathways th rough which oestrogen maintains endothelial nitric oxide production; the mechanistic distinctions among SERMs, SERDs and tissue - selective oestrogen complexes; and the gut microbiome oestrobolome, whose β - glucuronidase - mediated enterohepatic recycling gener ates clinically meaningful inter - individual pharmacokinetic variability. A narrative synthesis of 90 high - quality sources was conducted. Critical appraisal revealed that the existing evidence base is compromised by underpowered trials, short follow - up, eth nic homogeneity and absent pharmacogenomic stratification. The heterogeneous outcomes of postmenopausal hormone therapy are mechanistically explicable through convergence of ER subtype balance, metabolite profiles, co - regulator stoichiometry, eNOS coupling status and oestrobolome composition, mandating precision menopause medicine that integrates ESR1/ESR2 genotyping, CYP/UGT pharmacogenomics, oestrogen metabolite profiling, vascular biomarker - guided timing and microbiome characterisation into clinical prac tice.
