Abstract

Background: Asbāb-e-Sitta Ḍarūriyya, the Six Essential Determinants of Health in the Unani System of Medicine (USM), provide a foundational lifestyle-based framework for maintaining gastrointestinal GI) health. Traditionally described through the concepts of Mizāj, Akhlāṭ, and Quwwat-e-Hāzima, these determinants show strong alignment with contemporary understandings of GI physiology, neuro gastroenterology, circadian biology, and microbiome science. Objective: To present an integrated mechanism-of-action model linking traditional Unani determinants of health with modern biological pathways involved in digestive regulation. Methods: A narrative integrative review was conducted by synthesizing classical Unani literature with current biomedical evidence on digestion, metabolism, gut- brain signalling, microbial ecology, and lifestyle medicine. Conceptual mapping was used to identify converging pathways and construct a unified mechanistic framework. Results: Each determinant of Asbāb-e-Sitta Ḍarūriyya corresponded with key physiological processes governing GI health: Hawa, influence internal heat regulation and inflammatory responses, affecting digestive efficiency. Makulat-o Mashrubat, modulate humoral balance, gastric secretions, microbial diversity, and metabolic signalling. Harakat-o-Sukun Badani, enhances digestive heat, peristalsis, hepatic function, and detoxification capacity. Harakat-o-Sukun Nafsani, shape humoral dynamics and correlate with gut-brain axis regulation of motility, secretion, and visceral sensitivity. Nawm-o-Yaqza, restores digestive energy and aligns with circadian control of hormones, acid secretion, and microbiome rhythms. Istifragh wa Ihtibas, processes ensure the removal of morbid matter, paralleling modern detoxification and bowel function mechanisms. Conclusion: The integrated mechanism highlights substantial coherence between Unani principles and modern digestive physiology. Asbāb-e-Sitta Ḍarūryiya collectively maintain GI health by harmonizing temperament, regulating humoral production, strengthening digestive faculties, optimizing microbial ecosystems, and supporting neuroendocrine balance. This synthesis reinforces the relevance of Unani lifestyle determinants in the prevention and management of dyspepsia, GERD, IBS, constipation, and hepatic dysfunction.
