Deterioro de la función renal inducida por fármacos en paciente con poliarteritis nodosa
Abstract
We present the case of a patient with polyarteritis nodosa who, during her stay in a functional recovery hospital, experienced a deterioration in renal function, with a suspected pharmacological cause within a multifactorial context BACKGROUND Polyarteritis nodosa (PAN) is a systemic necrotizing vasculitis that affects medium-and small-sized arteries, producing secondary tissue ischemia in multiple organs, including the heart, where it may clinically manifest as heart failure (HF) [1].Acute deterioration of renal function (RF) in patients with this condition is a frequent and multifactorial event.Polypharmacy has been identified as one of the causes, with evidence showing that the combination of multiple medications together with other predisposing factors contributes to the multifactorial nature of these events. Therefore, prevention should focus on avoiding drug combinations that may precipitate renal injury [2].Appropriate drug selection, together with careful monitoring and an individualized therapeutic approach, is essential in these patients. CASE PRESENTATIONA 54-year-old woman with a diagnosis of multisystemic polyarteritis nodosa (PAN) developed ischemic heart disease secondary to vasculitis and HF in March 2001.
