Justification of the duration of preventive intake of vitamin D preparations
Abstract
In our opinion, the existing methodological materials regulating the duration of vitamin D supplementation need to be clarified, and the problem of determining the duration of the period of continuous use of vitamin D preparations in global clinical practice has not been sufficiently studied, and therefore the purpose of this study is to analyze changes in the concentration of calcidiol, calcitriol and paratherioid hormone in blood serum when taking supplements. vitamin D and the development of recommendations for regulating the duration of their use. We carried out statistical processing of the results of the use of vitamin D preparations (cholecalciferol, ergocalciferol and calcifediol), presented in 30 articles containing analysis data from 2,803 patients aged 49.5±16.9 years. A study of the results of a number of studies shows that when taking vitamin D supplements, at some point in time, the calcidiol content in the blood serum of most participants reaches a sufficient concentration, which cannot increase continuously and indefinitely, moreover, at some point in time, a decrease in the concentration of 25 (OH) often begins to occur despite the fact that vitamin D supplementation continues. Along with the stabilization or decrease in calcidiol levels, there is a stabilization or decrease in calcitriol levels, and the decrease in parathyroid hormone levels often stops and its growth resumes. It seems to us that when taking vitamin D supplements after reaching the optimal concentration of 25 (OH) D in the blood serum, which occurs no later than 6–8 weeks after the start of treatment, should not switch to maintenance doses, but temporarily stop taking vitamin D medications and take a break. The optimal duration of the break between courses of taking vitamin D preparations depends on many factors related to the characteristics of the human body, on the regimen of taking drugs, their type and quality, etc., However, with a certain degree of caution, it can be assumed that when the concentration of calcidiol in the blood serum reaches levels of 40–60 ng/ml, monthly Taking a break from medications can be useful both from the point of view of improving efficiency and from the point of view of saving money.
