Abstract

It is often said that the NHS can deliver only two of the following: access; continuity of care; and low cost.Given that most UK citizens support the founding NHS principle of 'free at the point of delivery', 1 how do we deliver access and continuity? 2 This Editorial outlines a vision for how access and continuity could be provided within existing resources. Some people will say 'it's too difficult'; 'it will cost more', 'it undermines the heart of general practice', and 'we will be left managing the most difficult patients'.However, the current model is unsustainable for staff, leaves patient needs unmet, and is increasingly unrewarding professionally.
