Ovarian hyperthecosis in a 62-year-old woman presenting with hyperandrogenism and elevated testosterone
Abstract
Ovarian hyperthecosis is a rare cause of hyperandrogenism in postmenopausal women, characterized by proliferation of luteinized stromal cells and excessive androgen production. We report in our article the case of a 62-year-old woman with progressive hirsutism, androgenetic alopecia, and deepening of the voice, associated with markedly elevated testosterone levels. Imaging studies revealed bilateral ovarian enlargement with stromal hyperplasia. The patient underwent bilateral oophorectomy, and histopathology confirmed ovarian hyperthecosis. Clinical improvement and hormonal normalization were observed postoperatively. Early diagnosis and surgical management of ovarian hyperthecosis lead to substantial improvement of symptoms. This condition should be considered in the differential diagnosis of hyperandrogenism in postmenopausal women.
