Abstract

Introduction. Breast cancer (BC) is the second most common cancer (after lung cancer), and BC ranks first among all malignant neoplasms (MNO) in women, demonstrating a tendency towards a constant increase in incidence. Breast cancer often metastasizes to distant organs, including the thyroid gland. Among all metastatic thyroid lesions, 7.8% of cases are consequences of breast cancer. Diagnostics of MNO in the mammary and thyroid glands presents certain difficulties. Despite screening programs, there is a high probability of obtaining false-negative diagnostic results. The fine-needle aspiration cytological puncture biopsy (FNACPB) is one of the fast, cheap, effective and comfortable methods for diagnosing palpable and ultrasound-observable lesions of the mammary and thyroid glands. Purpose. To analyze advantages of aspiration biopsy and cytological method of examining breast tumors with metastases to the thyroid gland. Materials and methods. The article considers a clinical case of a 61-year-old woman with a volumetric formation in the left mammary gland and a metastatic formation in the right lobe of the thyroid gland. Results. Despite the false-negative diagnosis of the tumor (according to ultrasound data) during the medical examination, the patient palpated a volumetric lesion in the left mammary gland by herself. Repeated ultrasound revealed a volumetric lesion in the left mammary gland with a final BI-RADS score of 5, as well as a hypoechoic nodular lesion in the right lobe of the thyroid gland with a TIRADS score of 4. The biopsy (thick needle aspiration) of the left mammary gland with cytological examination and FNACPB of the right lobe of the thyroid gland revealed carcinoma cells in both lesions. Conclusion. The clinical analysis performed allowed concluding that the fine-needle aspiration cytological puncture biopsy is an effective tool for diagnosing hypoechoic (cystic) volumetric lesions in the mammary gland with metastases to the thyroid gland.
