Abstract

The article traces the historical evolution of administrative process management models in Russian healthcare and explains how digitalization reshaped governance mechanisms at federal, regional and provider levels. The study applies a historical-genetic and comparative approach, content analysis of regulations and digital policy documents, and comparison with international digital health frameworks. The findings show a shift from a bureaucratic hierarchy (the Semashko model) to program-based management in the 1990s‑2000s, which paved the way for process management and management accounting, and further to a platform/ecosystem model in the 2010s‑2020s where data from the Unified State Health Information System and regional medical information systems becomes a managerial resource for monitoring and decision-making. Risks of ‘digital bureaucracy’ (administrative burden, fragmented IT landscape, data protection constraints) are identified, and conditions for higher managerial impact are proposed: interoperability, end-toend regulations, and a focus on reducing transaction costs of administrative procedures.
