Abstract

Nursing documentation is an important part of the delivery of healthcare services and supports the following: clinical communication, continuity of care, patient safety and regulatory compliance. However, the growing demands for documentation within electronic health record (EHR) systems have played a significant role in the creation of administrative burden, inefficient use of workflow, and professional burnout among nurses. In high acuity clinical environments, delays or inconsistencies in documentation may have an adverse impact on care coordination and patient outcomes. Recent progress in generative artificial intelligence (AI), especially large language models and natural language processing techniques, creates new possibilities to automate and augment the documentation of nursing and reporting from patients. This chapter looks at the role of generative AI-powered automation plays in the improvement of the accuracy, efficiency, and clinical utility of nursing documentation. Key applications discussed include automatic clinical document generation, voice enabled documentation, shift to shift handoff reporting, real-time integration of patient monitoring data and structured patient status summarization in an EHR system. The chapter goes on to discuss some of the methodological considerations of how to implement AI-assisted documentation such as fine-tuning AI models on de-identified clinical data, metrics for evaluation, and human in the loop validation to ensure clinical safety and accountability. Ethical, legal and regulatory issues, including data privacy, algorithmic bias, transparency and nurse responsibility are critically examined. Through the use of illustrative case scenarios based on empirical data, this chapter discusses the use of generative AI as a clinical co-pilot to support nurses without adding to the documentation burden while maintaining professional judgment. Overall, the chapter offers a mix of practical and scholarly insights for clinicians, healthcare administrations, and policymakers attempting to responsibly integrate generative AI into nursing documentation and clinical decision support.
