Extracellular Basophilic Material in Histopathology: A Pattern-based Diagnostic Approach with Case Illustrations
Abstract
Abstract Extracellular basophilic material (EBM) represents a common histopathologic finding with diverse etiologies, including protein deposition, mineral imbalance, mucinous neoplasms, fungal infections, and immune-mediated vascular injury. While readily observed on hematoxylin and eosin sections, accurate interpretation requires correlation with histochemical stains and clinical context. We present a case series illustrating five manifestations of EBM: (1) renal amyloidosis confirmed by Congo red birefringence and light-chain restriction, (2) metastatic calcification in end-stage renal disease confirmed by von Kossa positivity, (3) low-grade appendiceal mucinous neoplasm with mucin extravasation confirmed by Alcian blue staining, (4) cryptococcal infection with capsular deposits highlighted by Grocott methenamine silver positivity, and (5) leukocytoclastic vasculitis with fibrinoid necrosis confirmed by Masson’s trichrome. These cases highlight a pattern-based diagnostic framework, where morphology guides targeted use of special stains and directs appropriate management ranging from chemotherapy and metabolic correction to oncologic surveillance, antifungal therapy, and immunosuppression. This case series underscores the integration of morphologic, histochemical, and clinical data to transform a nonspecific histologic pattern into a precise, actionable diagnosis.
