Abstract

Transthyretin amyloidosis is a known cause of secondary glaucoma. The authors present a 3-year update on a previously published patient who underwent Kahook Dual Blade (KDB) excisional goniotomy in the right eye, followed by KDB combined with cataract surgery and vitrectomy in the left eye. Ab interno goniotomy with the KDB provided short-term intraocular pressure (IOP) control in the right eye. When combined with cataract surgery and a vitrectomy in the left eye, the patient achieved sustained IOP control at 3 years, with visual field stability. KDB combined with cataract surgery and vitrectomy to remove as much amyloid material from the eye as possible was effective in this patient with transthyretin amyloidosis-associated glaucoma. As such, this combination of procedures may be considered for the management of secondary glaucoma caused by transthyretin amyloidosis.
