Abstract

Ectopic atrial tachycardia (AT) with an underlying right bundle branch block is uncommon and may present significant diagnostic and therapeutic challenges, often mimicking other arrhythmias like fascicular ventricular tachycardia. We report the case of a 10-year-old girl who presented with persistent tachyarrhythmia and severely compromised cardiac function, requiring emergency radiofrequency ablation guided by advanced 3D electroanatomic mapping. This case emphasizes the importance of early electrophysiological intervention in pediatric patients with refractory AT, particularly when presenting with tachycardia-related cardiomyopathy and diagnostic ambiguity.
