A Case of Varicella-Zoster Virus Meningitis Presenting with Cluster Headache-Like Symptoms
Abstract
水痘・帯状疱疹ウイルス (VZV) 髄膜炎は皮疹を伴わないzoster sine herpeteで発症することがあり診断が困難である．46歳男性が激しい左側頭部痛・眼痛と同側の結膜充血，眼瞼浮腫，流涙を主訴に来院した．群発頭痛を疑いスマトリプタン投与と酸素療法を行ったが無効で，典型的な発作性経過も認めなかった．第3病日の脳脊髄液検査でVZV-DNA (2.1 × 104 copies/mL) を検出しVZV髄膜炎と診断，第7病日に三叉神経領域に皮疹が出現した．群発頭痛の標準治療に反応しない場合はVZV髄膜炎を鑑別に挙げ，早期の脳脊髄液検査が重要である．
