Abstract

This chapter covers the recurring patterns behind project failure, revealing that issues like missed deadlines, scope creep, and resource conflicts are often symptoms—not root causes. Drawing on cross-industry examples, it shows how uncertainty lies at the heart of most project struggles. Whether in shifting goals, unclear ownership, or reactive decision-making, uncertainty destabilizes execution. The chapter introduces the concept that successful innovation projects don’t eliminate uncertainty—they manage it. By recognizing these recurring patterns early, leaders can shift from reactive firefighting to proactive design, building strategies that anticipate ambiguity rather than fear it.
