Listening to a Whisper
Abstract
This opening chapter establishes the theoretical and ethical foundations for psychoanalytic psychotherapy practice, arguing that therapists must continuously strive to give verbal expression to all human experience, however resistant to articulation. Drawing on 34 years of clinical social work practice in Israel, the author explores the fundamental question: “what is it that I do when I do what I do?” The chapter presents an integrative framework combining Freudian drive theory with object relations approaches, emphasizing the interconnected nature of bodily sensations, emotions, and thoughts. Central concepts include the unconscious as a repository of early experiences that shape present behaviour, and repetition compulsion as both defensive pattern and creative hope for healing. The therapeutic relationship becomes a “laboratory” where unconscious patterns emerge through transference dynamics, understood here as bidirectional processes involving both client and therapist. Key theoretical contributions include a reconceptualization of trauma as primarily unconscious ruptures that resist symbolic representation, distinguished from painful but integrable losses. The chapter advocates for understanding symptoms not as pathology but as sophisticated survival strategies that once served protective functions. Drawing on Winnicott’s concepts of true and false self, it explores how defensive structures preserve the authentic core while enabling social adaptation. The chapter concludes with Salman Akhtar’s four-tiered listening framework – objective, subjective, empathic, and intersubjective – as an essential therapeutic skill. This hermeneutic approach positions psychoanalytic psychotherapy as a creative practice that searches for “traces of lost parts” of the self, ultimately serving the broader goal of individual and collective repair through the cultivation of mature ambivalence towards complexity.
