Abstract

Many countries have moved to a health and social care model of primary care, targeting community-level supports. In this book, we take a wide perspective on the term “community”. Equally, we support a multidimensional understanding of health and social care, which proposes a social determinant of health foundation with the application of person-centered principles to the community. Accordingly, we commence with a brief consideration of what constitutes a community. The World Health Organization (2025 :web) states that community is: …a specific group of people who share a common culture, values and norms, identity, interest, action, place, practice, or circumstance, and are arranged in social connection according to relationships which have developed over a period of time and may be modified in the future.
